Ombudsman reports serious hospital mistakes left baby brain damaged
The Parliamentary and Health Service Ombudsman reported on 29 October 2014 that a day old baby suffered permanent brain damage as a result of serious mistakes by a nurse and two doctors during an urgent blood transfusion.
The baby required the transfusion after developing severe jaundice. She deteriorated fast during the transfusion, developing difficulty breathing and her heart stopped. This collapse caused permanent brain damage and it is reported that the baby will be disabled and will require life long care. A nurse and two doctors made serious mistakes in relation to the transfusion and as a result more blood went out than was put in. They failed to maintain an even balance and this is what resulted in the baby’s collapse and brain damage.
The report indicates that Barts Health NHS Trust acknowledged the medical errors and the result of those errors and the Trust has written to the baby’s mother admitting the failures and apologising. The Trust also stated that a root cause analysis would be carried out, to find out why the failures in this case occurred and to take steps to ensure that they do not recur in the future.
