Ombudsman report highlights “shocking” NHS failures and medical errors

The parliamentary and Health Service Ombudsman published a report on the 29 October 2014 summarising 161 investigations the Ombudsman carried out this year.

In one case, a man died following a liver biopsy and it was found that there were various failures in his care, including inappropriate discharge from Accident & Emergency. Consent to the procedure not been properly obtained. Additionally, there was inadequate provision of monitoring and care and inappropriate medication was given.

There were several complaints about inappropriate discharges from hospital, failures in diagnosis of cancer, a baby was left brain damaged as a result of serious medical errors by a nurse and two doctors and a woman was left infertile and unable to have children, due to the devastating consequences of errors by NHS staff.

The ombudsman hopes that by publishing the summaries in the report valuable lessons will be learned by public services.
